Safe Design is not only our ethical responsibility, it is also quite simply, a measure of our success. A question, we as Engineers should always ask ourselves, is: how can we do this safer in design, construction, use and demolition. Underpinning all of this, is to create an environment where safety is paramount. How do we ensure those in an engineering team feel supported and free to share their mistakes and near misses with others instead of concealment? How can we raise the bar in safe design?
Design Office Pitfalls and How They Can be Avoided

As forensic engineers, we do not just look at the causes of disasters, we often communicate with the very people who made them possible.
